7/2/2023 EnchantaVaughn Approved QMHP-A greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-A inquired about her afternoon. Kaila verbalized to QMHP-A that her afternoon was going good. QMHP-A informed Kaila that this session was going to focus on managing her anger and being able to apply appropriate coping skills. Upon arrival, she did not present any symptoms of S/I. QMHP-A began the session by inquiring about Kailaâs current thoughts, feelings and emotions. QMHP-A engaged Kaila in a skills building activity. QMHP-A explained to Kaila that the purpose of this activity. QMHP-A explained to Kaila that the purpose of this activity was to assist her with how to effectively communicate with others and teach Kaila assertiveness skills to express angry feelings in a controlled manner and meet her needs through constructive actions through the use of role-playing and modeling. QMHP-A demonstrated different scenarios that displayed appropriate and inappropriate communication skills. QMHP-A discussed Kaila using âIâ statements to assert her feelings of frustration and stress. QMHP-A articulated the importance of Kaila using positive outlets to decrease her frustration or stress thereby reducing her anger. QMHP-A encouraged Kaila to use her journal to decrease her feelings of anger when she becomes frustrated or stressed. QMHP-A articulated other therapeutic ways to decrease her feelings of frustration or stress. QMHP-A advised Kaila to implement therapeutic techniques to reduce the frequency and intensity of anger during frustration or stress. Kaila appeared to be receptive to QMHP-Aâs interventions and services that were provided to her during the session. Kaila reported having a hard time using effective communication when she becomes frustrated or stressed. Kaila informed specialist she usually reacts to her anger by becoming verbally aggressive and sometimes physical. Kaila articulated how she has a hard time becoming assertive when she was frustrated or stressed. Kaila was able to identify the appropriate and inappropriate communication skills and was able to role-play both negative and positive communication skills. Kaila was able to use âIâ statement to verbalize her anger and address her feelings of frustration or stress. Kaila felt she would be able to utilize her journal and creative writing when she becomes frustrated or stressed instead of displaying anger outburst. Kaila agreed to use therapeutic techniques to reduce the frequency and intensity of anger outburst.
